We will provide new patient forms to complete prior to your visit. Bringing your photo ID, insurance card, and a list of current medications will help us get started quickly.
Your Consultation with Dr. Stapleton
Dr. Stapleton will thoroughly evaluate your condition, review any imaging or prior records, and discuss your diagnosis and treatment options in detail.
Your Personalized Treatment Plan
Together with Dr. Stapleton, you will establish a treatment plan — whether that involves physical therapy, a procedure, surgery, or ongoing management — tailored to your goals.

- Any prior imaging (X-rays, MRI, CT scans) — on disc or digital

- Please arrive 15 minutes before your appointment time
- Your initial visit typically lasts 45–60 minutes
- Dr. Stapleton will review your history and perform an examination
- We may take in-office X-rays during your visit
- A treatment plan and any necessary referrals will be discussed
- Our staff will assist with scheduling follow-up care
